    PARTICIPANT ACCIDENT WAIVER & RELEASE OF LIABILITY FORM:

    ARMED FORCES DAY POKER CHIP RUN and MOTORCYCLE GAMES ACCIDENT WAIVER & RELEASE OF LIABILITY FORM THEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING AND/OR VOLUNTEERING IN THIS EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

    I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event.

    In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

    (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event

    (B) I acknowledge that I am fully aware of the danger(s) and hazard(s) associated with participating in a motor vehicle ride. I certify that I am duly licensed and competent to operate a motorcycle in a safe and responsible manner, and the vehicle is in safe operational condition. I will be riding on public highways and understand that I am solely responsible to determine the speed and operational characteristics of my motor vehicle while participating in the run. I hereby release and hold harmless Fallon Community Events and any of its members, against any and all claims, causes of action, and/or any other liability of any kind arising from my participation and activity affiliated with this specific ride.

    (C) I certify that I have no known physical or mental impairment that may affect my safety or the safety of other participants. | understand and acknowledge that the choice of wearing a seat-belt, helmet and/or other protective gear is solely my responsibility. I am also responsible for my compliance with all state laws, including those regarding helmets. I certify that I am not under the influence of any narcotic, alcohol or other substance that may impair my understanding or judgment and that I will not at any time during this ride operate my motorcycle under the influence of any narcotic, alcohol or other substance that may impair me.

    (D) I understand that this Waiver and Release is in force from the start to the end of this event and covers any and all activities affiliated with the Fallon Community Events and/or this run. I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers, and assigns.

    The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

    I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
